Comments:
An unannounced monitoring inspection was conducted on 1/30/2023 from 10:25am to 11:20am. There were 32 school-agers directly supervised by 4 staff. The physical plant, 5 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in individual and group activities. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Description: Based on review, the facility did not ensure that they obtained documentation of an immunization record before the child attends the center.
Evidence: reviewed 5 children's records and found that Child 3 did not have documentation of an immunization record.

Description: Based on observation, the facility did not ensure that hazardous substances are kept in a locked place using a safe locking method that prevents access by children.
Evidence: observed that there was disinfectant spray, and Elixir spray accessible to children on a cart used by the cafeteria staff.
