Comments:
An inspection was conducted on March 25, 2022 concerning an unreported accident that was received from an outside source. There were 45 children along with 16 staff present. Violations observed during the inspection can be reviewed on the violation notice. Any questions contact nancy.radcliffe@doe.virginia.gov.
Based on interviews and photographic evidence, it was determined the center did not inform the department's representative as soon as practicable , but not to exceed two business days, of any injury to a child that occurred while the child was under the supervision of the center and required outside medical attention. Evidence 1. On March 8, 2022 at approximately 2:58 pm, a child under the supervision of the center, child #1 (17 months old) received burns to the skin from hot water that spilled onto their lower and upper left arm.
2. After 911 was called, the ambulance arrived at the center and transported the child to a local hospital.
3. The center never informed the department's representative of the injury within the required timeframe.
Plan of Correction:
Going forward we will contact our inspector to report the injury within the two business day requirement.

Description:
Based on interviews and photographic evidence, staff supervising children, did not always ensure their care, protection, and guidance. Evidence
1) Staff #1, Staff #2, and Staff #3 confirmed that on March 8, 2022:
A) At approximately 2:58 p.m., Child #1, (17 months old), while in the care of the center received burns to the skin on their upper and lower left arm. Child #1 received the burns from their contact with hot water, which was stored by staff in a vessel located on the table in the area where bottles are prepared.
B) The child was transported by ambulance to a medical care facility for treatment of the burns received.
2) The parent representative confirmed that on March 8, 2022:
A) At approximately 3:03 p.m., the parent of Child #1 was contacted by Staff #2 and was informed that while Child #1 was in the care of the center, they had received burns to the skin on their left arm from hot water contacting their skin.
B) The parent arrived at the center and called the Emergency number 911 at approximately 3:40 p.m. to seek medical assistance for the burn injury Child #1 received.
C) Child #1 was transported by ambulance to a local medical care facility for treatment of the burns received.
Plan of Correction:
A new policy was implement that there is to be no hot liquid of any kind in the classrooms or around the children. The parents are now brining in their own bottles of water to use for formula bottles.
