Technical Assistance:
Discussion that both TB documentation on file are set to expire 10/18/2023. Please renew before that date and place on file the new documentation.

Discussion about Medication Administration Training in regards to Child #5. The Provider expressed an interest in obtaining this certification.

Comments:
This monitoring inspection was initiated by licensing staff and concluded on 08/29/2023, between the hours of 9:30 am to 11 am. There were 4 preschool-age children present (8 points), with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication (household); special care and emergencies; and nutrition. A total of 4 children records, 1 staff file, and 1 adult household record was reviewed.

Information gathered during the inspection determined that there were (5) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the home.

Violations:
Standard #: 8VAC20-800-730-D
Description: Based on observation, household medication were not kept in a locked place.

Evidence -
1. On the date of inspection (08/29/2023), household medication was observed being stored in a small basket on a counter top within the kitchen of the FDH.
2. The kitchen is off the playroom of the childcare area and in-between that play area and the bathroom that the children in care can use.

Plan of Correction: Corrected on the date of inspection.

Standard #: 8VAC20-800-100-A
Description: Based on review, 2 of 4 children files indicated that the Provider did not obtain documentation of a physical examination (by or under the direction of a physician) within 30 days after the first day of attendance.

Evidence -
1. On the date of inspection (08/29/2023), documentation of a physical examination was not available for review for: Child # 4, and Child # 5.
2. The start dates for each were: 05/08/2023 for Child #4, and 04/18/2023 for Child #5.

Plan of Correction: Documentation of a physical examination will be obtained and placed on file for Child # 4. Child # 5's last day is today.

Standard #: 8VAC20-800-80-A
Description: Based on review, 1 of 4 children files indicated that the Provider did not obtain a child's proof of identification within seven business days of the child's first day of attendance at the Family Day Home (FDH).

Evidence -
1. On the date of inspection (08/29/2023), the proof of identification for Child # 4 was not available for review.
2. The start date for Child #4 was listed as 05/08/2023.

Plan of Correction: The proof of identification will be obtained and placed on file for Child # 4.

Standard #: 8VAC20-800-90-A
Description: Based on review, 1 of 4 children files indicated that the Provider did not obtain documentation that a child has been adequately immunized (according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations) before a child may attend the Family Day Home (FDH).

Evidence -
1. On the date of inspection (08/29/2023), an immunization page for Child #4 was not available for review.
2. The start date for Child #4 was listed as 05/08/2023. Child #4 was in attendance on the date of inspection.

Plan of Correction: An immunization page will be obtained and placed on file for Child #4.

Standard #: 8VAC20-800-280
Description: Based on observation and interview, cleaning agents, disinfectants, and deodorizers were observed in an area accessible to the children in care.

Evidence -
1. On the date of inspection (08/29/2023), cleaning agents, disinfectants, and deodorizers were observed either out on a shelf or within an unlocked cabinet located in the FDH bathroom that the children in care use.
2. The Provider stated (during an interview conducted on the date of inspection) that she has a lock for the cabinet, but it was not in place at the time of observation by the Licensing Inspector (LI).

Plan of Correction: Corrected on the date of inspection.
