Based on review of five children's records and verification from the provider, one child's record did not contain documentation that the child had received a physical examination.
Evidence: 1. The record for Child #4 did not contain a record for a physical examination. Date of enrollment for Child #4 was 05/13/2019. 2. The provider confirmed the physical exam report had not been obtained.
Plan of Correction:
The provider has talked with the parent of the child and will obtain a physical examination as soon as possible.
