Evidence: 1. While there was a paper towel dispenser affixed to the wall, the container was empty. A bath towel was present for hand drying. 2. Provider confirmed there were no paper towels present in bathroom. Plan of Correction: Paper towels had been ordered online and expected by the end of the week. In the future, will order early enough to receive prior to running out.
