Comments:
An unannounced monitoring inspection was conducted on 06/06/2023 from 10:05 a.m. to 10:45 a.m. in conjunction with local Child Protective Services in response to a self-reported incident. The self-report was received by the Department on 05/26/2023 regarding an incident that occurred on 05/25/2023. The inspector reviewed compliance in physical plant, staffing and supervision.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-560-A
Description: Based on interview, the provider failed to provide direct care and supervision of each child in care at all times, specifically awareness of and responsibility for each child in care, including being near enough to intervene if needed.
Evidence:
1. The provider reported to the licensing inspector on 05/26/2023 that on 05/25/2023 Child #1 had left the house through the back door, rode a Power Wheels tractor down the driveway and into the main road (Rt. 55/Strasburg Road).
2. A 45 second video, posted by a passerby on Facebook (FDH-1108924(06-06-2023)v1.mov), shows Child #1 getting off the Power Wheels tractor in the middle of the road, turning it around, and driving back down the middle of the road in the direction of the family day home. Cars are stopped on each side of the road.
3. The provider confirmed that she was not aware that the child had left the house and once she found Child #1, she was the end of the driveway, returning from driving in the road.

Plan of Correction: I have since purchased and installed door alarms on all doors that will alarm me when a the doorknob is touched. I did have the doors locked at the time of the incident, however, this was not enough to ensure safety. The tractor will also remain inaccessible during my Family Day Home hours.
