Based on interview, the provider failed to inform the department?s representative as soon as practicable, but no more than two business days, of any injury to a child while under the vendor?s supervision when a referral is made for treatment from a medical professional. Evidence: 1. On 05/30/2023, the Department received a complaint regarding an injury that occurred on 05/25/2023. 2.Interview with the vendor on 06/06/2023 confirmed that on 05/25/2023, Child #1 received medical treatment due to a knee injury that occurred from jumping off a piece of play equipment while Child #1 was under the supervision of the vendor. 3. The vendor confirmed that she failed to report the injury to the department?s representative.
