Evidence: 1. Documentation showed that the date of the last inspection of the fireplace was 03/01/2022 2. The provider confirmed that a fireplace inspection had not been conducted since 03/01/2022. Plan of Correction: I am working on an appointment for the inspector to come out. I will send the report when it is completed.
