Comments:
An unannounced monitoring inspection was conducted on 03/19/2024 from 2:50 p.m. 3:20 p.m. There were two children present, ranging in ages from eight to nine years old with one staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records and one staff records was reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of record, the provider failed to obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form every two years. Evidence: 1. The provider?s tuberculosis (TB) screening was dated 01/10/2022. 2. Household member #1?s TB screening was dated 01/10/2022. 3. The provider confirmed that the TB screenings had not been updated every two years.
