The Inspector reviewed 8 children?s records, 1 caregiver record and 2 pets, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on a record review, the vendor did not ensure all each enrolled child had all required information. Evidence: Child #4 and #5 did not have documentation of an immunization record, signed by the child's physician.
