Comments:
A monitoring inspection was conducted on 08/17/2023. There were 3 children present and 1 caregiver. A tour of the home was conducted, in addition to a record review and interviews with caregiver The Inspector reviewed 3 children?s records and 2 caregiver/household records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on record review, the provider did meet the criteria on the health and safety checklist at the time of the monitoring visit. Evidence: Child #3 did not have documentation of having a physical examination.
Plan of Correction:
The child had a physical, but did not obtain a copy. The provider has let the child's mother know that a copy is needed.
