Comments:
An unannounced monitoring inspection was conducted with the director beginning at 7:00 AM and ending at 8:25 AM. There were ten children in the care of two staff members. The children's ages ranged between five and eight years old. The children were observed drawing and doing arts and crafts. Eight children's records were reviewed. Two staff records were reviewed and in compliance. There were no medications on site to be reviewed. The first aid kit, flashlight, and weather-band radio were observed and in compliance. The most recent emergency escape drill was documented as 9/4/2019. The most recent shelter-in-place drill was documented as 9/3/2019. The most recent health inspection was dated 5/14/2019. The most recent fire inspection was dated 8/20/2019.

Based on a review of ten children's records, it was determined that not all of the children's records were complete. Evidence: Child A, child B, child C, child D, and child E's records were missing the work address and phone number for one of the parents.
Plan of Correction:
The director stated that she would get the missing information from the parents.
