Comments:
A code compliance inspection was initiated on August 3, 2022 and concluded on August 4, 2022. There were no children present. The Licensing Inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of three children?s records and two staff records were reviewed. Information gathered during the inspection determined non-compliance(s) with applicable code sections and violations were documented on the violation notice issued to the program.
Based on observation, facility did not have required written statement available to parents, guardians and the general public.
Evidence: Facility did not have the written statement regarding physical facilities, enrollment capacity, food services, health requirements for staff and public liability insurance available for parents, guardians and the general public.

Written statement with information will be posted at the entrance.
