Comments:
An unannounced monitoring inspection was made on 01/15/2020 related to a self-reported incident of forbidden actions that occurred on 01/13/2020 and was reported to the inspector on 01/13/2020. Video of the incident was reviewed and staff were interviewed. Based on the information gathered, the allegation is found to be valid and violations were cited related to the incident.
Based on review of video recordings and staff interviews, the center failed to ensure that behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.
Evidence:
1. Video footage from the center's video surveillance system on 01/13/2020 showed Child 1 getting up from the table during snack. Staff 1 roughly handled putting the child back into the seat. Child 1 is observed crying during the incident. 2. In another encounter, Staff 1 is observed clapping near the face of child 1 when the child would not stop crying and walking away from the table. The child reacted by crying. 3. Staff 2 verified that Staff 1 clapped in/near the child's face. The center did not ensure behavioral guidance was constructive in nature and age and stage appropriate.

Description:
Based on review of video recordings, a letter from a staff member, and staff interviews, the center failed to ensure that physical punishment, striking a child, roughly handling or shaking a child, or other forbidden actions shall not used as a form of punishment.
Evidence:
1. Video footage from the center's video surveillance system captured Staff 1 roughly handling a 19 month old on 01/13/2020. Child 1 was observed getting up from the table during stack. Staff 1 roughly handled, putting the child back into the seat. Child 1 is observed crying during the incident. 2. Staff 2 stated that Staff 1 "put the child back in their chair aggressively." 3. In a written statement, Staff 1 stated he sat Child 1 "hard in his chair".
