Comments:
Licensing Inspector arrived at 10:15 and departed at 11:00 AM to conduct a religiously exempt code compliance inspection. At the time of the inspection, there were no children present as it was a teacher work day. Staff and principal verified that there are 11 preschool aged children enrolled who attend daily. All spaces utilized by the preschool children and the before & after school aged children were inspected. An exit meeting was held with the office manager and the principal to review the findings of the inspection.
Based on review of four staff records, it was determined that the facility did not ensure that all staff had a completed search of the central registry on file within 30 days of employment.
Evidence: The record for staff #2, date of hire 9/3/20, did not have a completed search of the central registry (CPS) on file.
Plan of Correction:
Central registry check will be sent for and results placed in record.

Description:
Based on review of four staff records, it was determined that the facility did not ensure that all staff all staff who have lived outside of Virginia within the past five years had a completed search of the central registry for those states
Evidence: 1. The sworn statement for staff #2 indicated that they had lived in Indiana in the past five years. There was no search of the central registry for Indiana on file. 2. Staff #3 stated that an Indiana search of the central registry had not been submitted.
Plan of Correction:
Search of the central registry for Indiana will be submitted.
