Comments:
An unannounced monitoring inspection was completed on 8/29/19. There were 42 children present in four groupings. Five children's records and eight staff records were reviewed. Five medications were reviewed. The inspector arrived at the center at 10:10 am and departed at 12:45 pm. The children were observed during outside times, during structured large group time, during lunch, and during nap. All infant activities were also observed.

Description: Based on observation, the center failed to ensure that areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin. Evidence: 1. There were several (approximately five) metal rebar stabs that were protruding from the landscape timber barriers around the top preschool playground. The metal stabs were nailed into the wooden landscape timbers to anchor them to the ground. The metal stabs were protruding from the top of the landscape timbers and presented a potential fall/trip and puncture hazard. The stabs were protruding approximately 1/4 inch up to one inch above the landscape timbers. The children were observed during the inspection to walk on the timbers and play near the timbers with the protruding stabs.

Plan of Correction: This was corrected during the inspection. The site administrator hammered the stabs down into the landscape timbers so they were no longer protruding.
