Comments:
The local agency representatives and licensing inspector conducted an unannounced complaint inspection on 02/11/2020 between 2:00 pm to 3:00 pm in response to a complaint that was received by licensing on 02/03/2020 related to allegations in the area of programs and staffing and supervision of the licensing standards. During the inspection, the licensing staff conducted interviews with staff, completed observations at the center, reviewed staff and children records, and other pertinent documentation. Based on the evidence gathered during this investigation it was determined that there was a violation of subsidy requirements for reports. See the violation notice for more details. Based on interview, the center did not inform the department's representative as soon as practicable, but not more than two business days, of any injury to a child who while under the vendor's supervision when a referral is made for treatment from a medical professional. Evidence: During the inspection on 02/11/2020, interviews confirmed knowledge of the incident on 02/03/2020 involving Child#1, age 4 at the center requiring medical treatment and the center not informing the department's representative within required timeframe.
Plan of Correction:
The owner/director will inform licensing within two days of any treatment, when it is dealing with medical.
