Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 3/23/21 and concluded on 3/25/21. The provider was contacted by telephone to initiate the inspection. There were 12 children present with 2 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on record review and interview, the provider failed to obtain updated central registry background check and sworn statement every five years for one staff.
Evidence:
1. The central registry for staff 1 was dated 10/21/15. The sworn statement for staff 1 was dated 11/17/15. 2. The provider stated she was unaware of the requirement for updating background checks and sworn statements.
Plan of Correction:
The provider updated the sworn statement and forwarded the sworn statement to licensing on 3/25/21. The paperwork for the central registry will be completed and mailed. In the future all staff background checks and sworn statements will be updated every five years.
