Comments:
An unannounced renewal inspection was conducted on 10/4/19 from 9:40am - 10:40am. At the time of the inspection there were two children in care, ages two years old (4 points), with one caregiver present. Records were reviewed for two children, four caregiver/household members, and one pet. Children were observed playing with toys. The provider stated there are no medications being administered at this time. Emergency plan and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice and were discussed with the provider during the exit interview.

Violations:
Standard #: 22VAC40-180-100-C
Description: Based on a review of two children's records, it was determined that the provider did not ensure that an individual record is maintained for each child enrolled with all of the required items.

Evidence:
1. The record for child #1, present during the inspection did not contain a copy of a physical examination, the proof of identity and the information for parents statement.
2. The record for child #2, present during the inspection did not contain the proof of identity and the information for parents statement.
3. The provider reviewed the records and confirmed they were not complete

Plan of Correction: The provider responded: The parents of each child will be asked to provide any information that is missing or needs to be updated. Newly enrolled children will have a complete record before starting.

Standard #: 22VAC40-180-120-C
Description: Based on observation, and a review of the health and safety checklist, it was determined that the provider did not meet the criteria on the health and safety checklist.

Evidence:
1. The floor in the hallway that leads to the bathroom in the house is torn and could be a tripping hazard.
2. The provider confirmed that the floor was not in good repair..

Plan of Correction: The provider responded: The floor will be replaced..
