Comments:
An unannounced monitoring visit and facility tour was conducted on 5/18/2022. At the time of inspectors' arrival there were 54 infant to preschool aged children in care with 10 staff members and 2 administrators. Children were observed interacting with staff, nap, lunch and hand washing. A sample of 5 children's records and 5 staff records were reviewed. Injury reports were also reviewed. The director reported no medications are being administered at this time. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Description: Based on observation and interview it was determined the center did not ensure that all hazardous substances are kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. There were chemical cleaning agents in the unlocked closet in 2 year old room.
2. Staff confirmed that the closet was unlocked during the inspection.

Plan of Correction: Staff will ensure that all hazardous substances are kept locked.

Description: Based on record review and interview it was determined the center did not request at least annually parent confirmation that the required information in the child's record is up to date.
Evidence:
1. The records for child #1 and child #2 did not contain documentation of annual parent confirmation that the required information in the child's record is up to date.
2. Staff confirmed that the records did not contain documentation of annual review.

Plan of Correction: Center will ensure an annual parent review of children's records is documented.
