Technical Assistance:
Technical assistance was provided regarding required emergency contact information for staff. Consultation was also made regarding various topics including enrollment, hiring processes, and the physical plant.

Comments:
An unannounced monitoring inspection was conducted today from 9:00am-1:40pm. There were 107 children directly supervised by 19 staff and all classrooms were in ratio. The physical plant, outdoor playground area, programming, menus, classroom attendance, 5 staff records, 11 children's records, 4 medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed listening to story time, participating in circle time, singing a "welcome" song, painting, playing with building blocks, doing an art activity and learning the letter "K", and learning colors in Spanish. Infant care including feeding and diapering was also observed during this inspection. There was an abundant supply of age appropriate materials for the children and the center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Description: Based on record review, 5 of 11 children's records reviewed did not contain all required information.

Evidence: Child #2, Child #5, Child #6, Child #7, and Child #8's records did not contain documentation of an annual review of emergency contact information completed by parents. The most recent annual reviews were completed on 8/17/18, 8/21/18, 11/18/18, 8/22/18, and 8/23/18, respectively.

Plan of Correction: We will be sending an updated emergency contact form to all families to complete. It will be reviewed by families on an annual basis to ensure emergency contact information is up to date.

Standard #: 22VAC40-185-510-A
Description: Based on review of 4 medications and medication authorization forms, it was determined 1 medication was on site without signed parental authorization to administer the medication.

Evidence: Child #12 has an emergency medication on site and does not have a signed parental authorization granting the center permission to administer the child's medication.

Plan of Correction: We will have the parent sign off on the medication authorization form.
