Comments:
An unannounced monitoring inspection was conducted from 10:30 am - 2:00 pm. There were 76 children in care being supervised by 14 staff. The children were observed eating lunch and preparing for rest. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of six child records and five staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of five staff records it was determined that not all records were complete. Evidence: The record for Staff A (start date: 05/30/2023) did not contain documentation of at least two references on file as required.

Administration will request two references for staff A and submit them for the file.

Description:
Based on review of five staff records it was determined that not all staff completed the Virginia Pre-Service Training within 90 days of employment. Evidence: The record for staff A (start date: 05/30/2023) did not contain documentation that staff A had completed Virginia Pre-Service Training.

Staff A will complete Pre-Service Training and submit the certificate to administration.

Description:
Based on interviews and review of documentation it was determined that there was not always someone on site who had current Medication Administration Training. Evidence: Staff B is the only person on site who has current Medication Administration Training. The facility is open from 6:30 am - 6:00 pm and staff B's schedule does not cover the full hours that the center is in operation. Currently, there are children enrolled who require emergency medication such as epinephrine.
Plan of Correction:
Administration will schedule a Medication Administration Training (MAT) course and enroll enough staff to ensure that there is always someone on site during operating hours who is MAT certified.

Based on review of medication it was determined that not all medication was sent home within 14 days after the expiration of the medication authorization form. Evidence: The emergency authorization forms for two medications for child A expired 05/19/2023.

Description:
Based on interviews and review of documentation it was determined that practice evacuation drills were not conducted monthly. Evidence: The most recent documentation that a practice evacuation drill had taken place was conducted on 04/20/2023.
