Comments:
An investigation was initiated from 12:15 p.m. - 3:45 p.m. in conjunction with a monitoring inspection to investigate a complaint received by the department involving supervision and procedures for emergencies. Interviews were conducted, documentation, staff records, and children's records were reviewed. There was not enough corroborating evidence to support the portion of the complaint involving procedures for emergencies. The portion of the complaint regarding supervision is determined to be valid.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description: Based on staff interviews and review of documentation it was determined that staff did not always ensure the care protection and guidance of children under their supervision. Evidence: Based on staff interviews it was determined that administration did not provide support to staff in need of assistance in classrooms who have children enrolled with challenging behaviors. Staff report that there are two children enrolled in the Pre-K classroom with challenging behavior. It is reported that staff are unable to manage and supervise the entire classroom while alone due to focusing their attention on the children with challenging behaviors. Staff are reported to be alone during rest time and in the afternoons around 4:00 pm - 6:30 pm. Staff have reported these concerns to administration however, they are still caring for children alone once the ratio allows for one teacher without support. The Licensing Inspector observed 11 injury reports from 08/03/23 - 02/13/24 where injuries were sustained to children by other children in the Pre-K classroom.

Plan of Correction: The two children who at times display challenging behaviors, have been separated into two classrooms.

One of the children who at times displays challenging behaviors is under the direct care of a developmental early intervention team, is fully participatory in an IEP , and is directly supported while at the school by a paraprofessional for the majority of the day. The parents of this child are actively engaged in pursuing additional support for the care of the child when the paraprofessional is not in the classroom. School administration continues to support the classroom teachers when alone in the classroom and the para is not present. Classroom staff have been reminded to use support tools in place for this child and to alert administration immediately when additional support is needed.

The 11 injury reports over the 6 month time frame were not solely related to the two children who at times display challenging behaviors, nor were all injury reports the result of challenging behaviors. Administration continues to monitor all
injury reports.
