Comments:
An investigation was initiated from 2:00 p.m. - 2:45 p.m. in response to a complaint received by the department involving supervision, and procedures for emergencies. Interviews were conducted, documentation, staff records, and children's records were reviewed. There was not enough corroborating evidence to support the portion of the complaint regarding procedures for emergencies. The portion of the complaint involving supervision is determined to be valid.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Complaint related: Yes
Description: Based on review of documentation it was determined that staff did not always ensure the care, protection, and guidance of children under their supervision. Evidence: Based on injury reports and incident reports for the Pre-K classroom it was determined that child A (Age 5 years) injured children and staff eight times between the dates of 02/23/24 - 03/06/2024. The behaviors that occurred in these reports include hitting, kicking, punching, and hitting with large blocks and puzzles in the classroom.

Plan of Correction: At the d1rect1on of the licensing specialist, the classroom staff have been specifically recording Child A's behaviors that are challenging in an effort to focus support efforts. Additionally, several strategies in line with the child's IEP are in place within the classroom, and staff are continually supported by school administration. School administration continues to work cooperatively with the child's developmental intervention team to continue to improve outcomes.
