Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A complaint/monitoring inspection was initiated on 9/17/2021 and concluded on 10/6/2021. A complaint was received by the department regarding allegations in the areas of Staff to Child Ratios and Supervision.
The evidence gathered during the investigation did not support the (allegation(s)/self-report) of non-compliance with standards or law. Any violations not related to the (complaint(s)/self-report) but identified during the course of the investigation can be found on the violation notice.
Based on review, the facility did not ensure that staff records contained the required minimum information. Evidence: Reviewed 4 staff records and found that 4 did not have complete documentation of reference checks. Reference checks must contain dates of contact, names of person/firm contacted, results and signature of person making the call. None of the references had dates of contact and signature of the person making the call.
