Comments:
A annual Religious exempt inspection was completed on September 20, 2023 from 9:45am - 11:16am . There were 47 children and 8 staff and additional administrative staff was present. During the inspection children were observed children outide playing and freely playing. Six staff records were reviewed.
Information gathered during the inspection determined non-compliance(s) with applicable requirements and violations were documented on the violation notice issued to the program.
Based on record review and staff interview, the licensee did not ensure every five years staff under go background checks.
Evidence: The following background checks are more than 5 years old:
Staff #1's most recent central registry was 09/14/2018, fingerprints 06/11/2018 and sworn statement was 06/11/2018. Staff #2 's most recent central registry was 07/13/2018 and fingerprints 05/22/2018. Staff #6's most recent central registry was 07/13/2018, fingerprints 07/13/2018 and sworn statement was 05/16/2018.

Description:
Based on record review and staff interview, the facility did not ensure to obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
Evidence: Staff #5 employment date 07/06/2021. Staff #5 lived in Washington in the past 5 years. The center was unable to provide Washington state criminal history record report and Child Abuse and neglect registry check.
