Comments:
An unannounced monitoring inspection was conducted today from 12:10pm to 2:20pm with the provider and one assistant. There were 10 children in care, ranging from 14 months to four years of age, for a total of 26 points. Children were observed finishing lunch, using the restroom, washing hands, and laying down for nap. Lunch today was chicken and broccoli macaroni and cheese bake with fruit cocktail and milk. Five children's records, two staff records, and one household member's record were reviewed. The provider reported that there is no medication for children at this time but she is current in Medication Administration Training (MAT). The first aid kit, flashlight, and weather band radio were observed. Based on review of documentation and interview with the provider, emergency evacuation procedures were not practiced monthly. Evidence: There were no evacuation drills documented for the months of June, July, August, September, or October and the provider stated that the documentation was accurate.

I will make sure to do evacuation drills monthly moving forward and document them.

Description:
Based on interview with the provider, the family day home did not keep a written record of children in attendance each day. Evidence: The provider stated that she does not keep a written daily attendance.
Plan of Correction:
The parents normally sign their children in and out themselves, but I will begin keeping a written attendance as well.

Description:
Based on review of two caregivers records, the family day home did not ensure that a caregiver was present who has current certification in cardiopulmonary resuscitation (CPR) issued within the past two years. Evidence: The last documented CPR certification for Caregiver A was dated 6/24/17 and the last documented CPR certification for Caregiver B was dated 10/9/17.
Plan of Correction:
We have already scheduled to be re-certified on 11/7/19.
