Comments:
An unannounced monitoring inspection was conducted today between the hours of 8:15 am through 9:20. There were a total of 7 children (15 points) observed with 1 caregiver. A total of 3 children's files and 2 staff/household member's files were reviewed. The children were engaged in the following: breakfast, diapering, blocks, manipulatives and hand washing. Based on a review of children records, the provider has not maintained a complete record for each child. Evidence: Child #1, date of first attendance, 6/24/19 does not have documentation of proof of identity on file.
