Comments:
A follow-up monitoring inspection was conducted today, February 9, 2023. The inspector was on site from approximately 12:55 pm until 2:00 pm. The director was not present at the time of the inspector?s arrival but arrived later. There were 31 children present, ranging in ages from three months to five years, with five staff supervising. Children were observed napping. In the Infant room two children were being feed while the others napped and were rocked in bouncers. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on observation, the center did not ensure all areas of the center, inside and outside, were maintained in a clean, safe, and operable condition.
Evidence:
Two pickets on the wooden fence enclosing the playground were broken. The pickets had splintered wood creating jagged points that could snag the skin of a child.

Description:
Based on observation and interview, the center did not ensure prepared infant formula was refrigerated, dated and labeled with the child?s name.
Evidence:
Two prepared bottles were observed setting on the feeding table in the infant room. The two bottles were not labeled with a name or date. An additional prepared bottle was observed on top of the refrigerator, without a name or date.
