Comments:
An unannounced monitoring visit was conducted today, Four children were present with one caregiver for eight points which is within ratios for supervision. Observed children finish eating lunch (Chicken and rice, broccoli, applesauce and milk), brushing their teeth, transition to outside play in the back yard and hand washing. Reviewed 2 records, emergency preparation, training and medication that was on-site for one child. Provider has current Medication Administration Training (MAT ) certification until 11/2024. Home is very well maintained, equipped with ample toys and equipment for young children and organized for children in care. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Feel free to contact me if you need any assistance.

Based on observation, an authorization for longer term medication has expired. Evidence: Medication authorization for Child#1 did not include a parent authorization and the physician authorization expired 2/23/2023.
Plan of Correction:
Will request the parent and physician complete new forms for long term medications.
