Comments:
A complaint inspection was initiated on 4/11/2024 and concluded on 5/28/2024. There were three children present with two staff supervising. The inspector reviewed compliance in the areas of staffing and supervision, special care. A total of two staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on observation the center failed to ensure that an out of state Sex Offender Registry completed for each state an individual has resided in the past 5 years prior to the first day of employment/service. Criminal History name check is required prior to he first day of employment or service. Evidence: Staff #1 (hire date8/19/23) has resided in another state in the past 5 years. Staff #2 (hire date 3/25/24) has resided in another state in the past 5 years.

Description:
Based on record review the center failed to ensure that each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children. Staff #2 (hire date 3/25/24) did not have a TB screen on file.
