Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).
A monitoring inspection was initiated on December 2, 2021 and concluded on December 7, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 2 children present, ranging in ages from birth to 12 years with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on review of two children's records, the provider did not obtain before the child attended documentation that the child has been adequately immunized according to the requirements of the Code of Virginia for two records. Evidence The records for child #1 and #2 do not contain documentation that the child has been adequately immunized according to the requirements of the Code of Virginia.
Plan of Correction:
The mom said she will bring them in this week, I will make sure to have them by Friday. In the future, I will get an immunization record before the child's first day.
