Arrived 10:05am Departed pm A Subsidy Health and Safety inspection was conducted on 10/15/2019 to review supplemental health and safety requirements. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Thank you for your cooperation. Based on the review of seven staff files, the facility failed to ensure that all staff have completed a TB test or screening prior to coming into contact with children. Evidence: 1. There are no TB test results on file for staff #3 (hired 8-5-19), #5 (hired 9-9-19), #6 (hired 4-22-19).
Plan of Correction:
Staff #4 is having a TB test today. Staff #5 had a TB test this morning. Staff #6 will have the results of the TB test read tomorrow 10/16/19.
