An unannounced monitoring inspection was conducted today.
There were 66 children present with 18 staff.
The children were playing in centers, eating lunch, playing outdoors and resting.
Five child records and six staff records were reviewed.

Standard #: 22.1-289.035-B-4
Description: Based on the review of six staff records, the facility failed to ensure that all staff complete all required background checks within the required timeframe.
Evidence:
1. There is no out-of-state central registry check on file for staff #3. (hire date 8-24-22)
REPEAT VIOLATION

Plan of Correction: The owner reported that the request for a search of the central registry was sent to the state of previous residence, but has not been returned from the central registry.

Standard #: 8VAC20-780-330-B
Description: Based on a review of the playgrounds, the facility failed to ensure that resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus.
Evidence:,
1. The resilient surface material was measured at 1-4 inches, of the required six inches, on all playgrounds.
REPEAT VIOLATION

Plan of Correction: The owner reported that more rubber mulch will be ordered and distributed to ensure that all areas contain 6 inches of resilient surface material.

Standard #: 8VAC20-780-510-L
Description: Based on a tour of the classrooms, the facility failed to ensure that all medication is locked.
Evidence:
1. There were three staff medications in an unlocked upper cabinet in the three's classroom.

Plan of Correction: The medication was locked during the inspection.
