There were 110 children present with 20 staff. The children were eating breakfast, playing incenters, having group time and playing outdoors. Five child records and eight staff records were reviewed. Violations: Standard #: 8VAC20-780-330-B Description: Based on a tour of the playgrounds, the facility failed to ensure that the resilient surface material complies with minimum safety standards and is placed under equipment with moving parts or climbing apparatus. Evidence: 1. The resilient surface material was measured at 1-4 inches, of the required six inches, on all playgrounds. REPEAT VIOLATION Plan of Correction: Not available online.
