Technical Assistance:
Technical assistance today included indoor and outdoor areas, potential access to items, medication authorizations, working with multiple age children, and background checks for when a family member turns 18. (Fingerprint, Search of the Central Registry and Sworn Statement).

Comments:
An unannounced monitoring inspection took place from 10 am - 12:30 pm today. There were 6 children in care for 14 points with the provider and assistant. I observed children engaged in role playing, doing finger plays and singing, listening to a Little Red Riding Hood tape and imitating the characters; eating lunch, and reading stories before nap. Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records today, some items were not documented as required.
Evidence:
There was no documentation of updates of emergency contact information for Child # 2 ((due 6/19) and Child # 4 (due 6/19)

Plan of Correction: I will have parents review, make changes and sign.

Standard #: 22VAC40-111-90-C
Description: Based on a review of records today, one child's immunizations were not up to date as required.
Evidence:
Child # 5 (age 13 months) did not have documentation of immunizations every six months. Only one type of immunization series was documented, with no further information available today.

Plan of Correction: I will talk to the parent today.

Standard #: 22VAC40-111-240-A
Description: Based on observation during today's inspection, there were some areas outdoors that were not in safe condition.
Evidence:
There was a door stacked sideways and construction material in areas accessible to children during the outdoor picnic snack..

Plan of Correction: My husband is working on the outdoors areas. He will be putting down turf by this weekend. We will avoid the areas until it is fixed.

Standard #: 22VAC40-111-530-F
Description: Based on observation during today's inspection, a pillow was under a child under age two.
Evidence:
Child # 5 (13 months old) was observed in a crib with a pillow under the head today.

Plan of Correction: The parent requested the pillow because of the child's snoring. I will ask her to get a doctor's note to do this.
