Comments:
An unannounced monitoring inspection was conducted today from 12:30 pm until 3:00 pm. There were 9 children present (24 points) with 3 caregivers. The children were observed playing with toys, singing, during nap time and during infant feeding.
The physical plant, emergency supplies, the emergency plan, menu, attendance logs, evacuation logs children's records, caregivers's records and household member's records were reviewed. There was no medication on site during today's inspection. Areas of non compliance are cited on the Violation Notice. Based on observation, the family day home failed to ensure that the first aid kit contains minimum supplies.
Evidence:
The first aid kit is missing adhesive tape.
Plan of Correction:
I will purchase and put in first aid kit.

Description:
Based on a review of 5 children's records, in 1 out of 5 children's records, the center failed to ensure that the child had a physical by the 30th day of attendance.
Evidence:
Child #4's file does not have documentation of a physical by the 30th day of attendance.

Description:
Based on review of 5 children's records, in 1 out of 5 children's records, the family day home failed to maintain up to date records for each child enrolled.
Evidence:
1) Child #1's file did not have documentation of a medical policy number or information for the second emergency contact.

Description:

I will take dog to vet and get updated rabies vaccine.
