Comments:
An unannounced non-mandated inspection was completed on March 13, 2020 from 10:50 A.M. ? 12:00 P.M. The inspection was conducted in response to a self-reported incident that occurred at the center on February 25, 2020. Two children records, two staff records, staff documentation, and policies and procedures were reviewed. Three staff members were interviewed and a staff member accompanied the licensing inspector to the classroom to review the events that occurred. The exceptions to compliance are noted on the violation notice.

Violations:
Standard #: 22VAC40-185-340-A
Description: Based on interview and review of center documentation, it was determined that the center failed to ensure that when staff are supervising children, they ensure the care, protection, and guidance.

Evidence:
1. On 02/25/2020 at approximately 10:15 A.M., Staff #2 was going on lunch break and shared with Staff #1 who needed to be fed next.
2. Staff #2 went to break and Staff #1 mistakenly took out Child #2?s breast milk from the freezer to feed Child #1. There was approximately 2.5 ounces of milk in the bag.
3. Staff #1 warmed the milk and gave Child #2?s breast milk to Child #1.
4. At approximately 11:00 A.M. Staff #2 returned from break. She noticed a used breast milk pouch in the trash from the freezer that was for Child #2. Staff #2 checked the daily reports and found that Child #2 was not fed but Child #1 had been fed even though his breast milk was still in the freezer.
5. Staff #2 contacted Staff #1 and verified that Child #1 had consumed Child #2?s breast milk, and there had been a mistake. Staff #2 notified Staff #3 at approximately 11:15 A.M.

Plan of Correction: We started with putting a consistent person to break those teachers, who know the children and their names. I also told staff if they are unsure about something to ask the director.
