This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).
A complaint inspection was initiated on 11/15/2021 and concluded on 11/15/2021. The director/owner was contacted by telephone and an investigation was conducted. Two interviews were conducted with staff. The information gathered during the investigation does support the allegation, so the complaint is determined to be "valid". The exceptions to compliance are noted on the violation notice.
Please complete the plan of correction and date to be corrected sections for each violation notice and return it to me within 5 business days from today. Please specify how the deficient should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Based on interview and verification from staff the center did not ensure that aides were at least 16 years of age.
Evidence: 1. Staff #1 stated that Staff #3 was currently employed by the center as an assistant. 2. Staff #2 verified that Staff #3 was employed and currently worked as an assistant in different classrooms the afternoons.
Plan of Correction:
Employee will no longer be working as a teacher's aide, but will only be on staff for cleaning purposes.
