The provider has submitted an initial application to the Licensing Office and is currently working with the Local Zoning Administration to complete the Contact with Local Zoning Administrator form prior to the issuance of the new license.

An announced inspection was completed on 1/30/2023 in response to a planned relocation of the licensed program. There were no children in care during the inspection. A total of 5 children's records, 2 household member's records, and 1 staff record were reviewed. There was an adequate supply of books, toys, and materials for the children. The home was clean and organized. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
