Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A subsidy health and safety inspection was initiated on April 15, 2021 and concluded on April 21, 2012. The center owner was contacted by telephone to initiate the inspection. There were 9 children present and 3 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection. The information gathered during the inspection determined non-compliance(s) with subsidy regulations, and violations were documented on the violation notice issued to the facility.
Based on record review and staff interviews, the vendor did not ensure staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department. Evidence: The record for staff #1 did not include a TB screening prior to employment. Date of employment 2-15-21. Center owner confirmed missing information.
Plan of Correction:
Staff member is scheduled for a TB test May 18, 2021. Hampton Health District. I did not realize the health record hold during the pandemic had resumed.
