Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews) and a virtual tour of the program. A monitoring inspection was initiated on 8/24/2021 and concluded on 8/24/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 5 children present, ranging in ages from 2 years to 4 years, with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 3 staff/ household records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on a review of children records, the parents have not been provided all the required information before the child's first day of attendance. Evidence: Child #1, date of first attendance 6/8/2021 and child #2, date of first day of attendance 6/7/2021 records do not have policies for the administration of medications.
Plan of Correction:
The provider will have the parents complete the form.

Description:
Based on a review of children records, before a child may attend the family day home, the provider has not obtained documentation that the child has been adequately immunized. Evidence: Child #1, date of first attendance 6/8/2021 does not have documentation of the child's immunization record.
Plan of Correction:
The provider will ask the parent for the immunization record and place in the child's record.
