Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated and concluded on April 8, 2021. The provider was contacted by telephone to initiate the inspection. There were seven children (15pts) present and two staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed two children?s records, one household member's record, and two staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-191-40-D-4-a
Description: Based on record review, a household member did not have documentation of a completed Central Registry Findings.

Evidence:
1. Central Registry Findings are good for 5 years. Household Member #1's record contained an expired Central Registry Finding. The Central Registry Finding was dated 03/224/2016.

Plan of Correction: The Household Member will complete a Central Registry Search and mail it. Once the findings are returned, the provider will send a copy of the results to the licensing inspector.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, 1 of 2 caregiver records did not contain documentation of a completed central registry findings.

Evidence:
1. Central Registry Findings are good for 5 years. Staff #1's record contained a Central Registry Finding that was expired. The Central Registry finding was dated 03/24/2016.

Plan of Correction: Provider will complete a new Central Registry Search and send the licensing inspector a copy of the findings when they come back.
