An unannounced monitoring inspection was conducted today between 10:00 am and 12:30pm. There are six children enrolled; however, no children were present today. Physical plant, children's records, caregiver's records, household member's records, emergency plans, evacuation logs, cribs and emergency supplies were inspected. Based on review of children's files (1 of 3), the family day home provider failed to maintain up to date records for each child enrolled.
Evidence:
Child #2's file is missing documentation for the second emergency contact.
Plan of Correction:
I will get information from parent and put in child's file.
