Comments:
An unannounced monitoring inspection was conducted on 05/09/2024 from 9:55 a.m. to 10:40 a.m. There were seven children present, ranging in ages from three to four years old with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of one staff records and four child record were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of record and interview, the provider failed to review the parent emergency contact information and obtain the parent?s signed acknowledgment of the review at least annually. Evidence: 1. Child #3, Child #4 and Child #5?s record did not have documentation of an annual review of the emergency contact information. Staff #1 confirmed that the emergency information had not been reviewed annually.
