Comments:
An unannounced on-site mandated inspection was conducted on 02/28/2024. There were 42 children, ages 7 months-2 years, and 8 staff members present during the inspection. The inspector observed the following: nap, and afternoon snack. A total of 4 children?s records and 6 staff records, were reviewed. The inspector discussed the following with staff: attendance requirements, returning staff. The inspector arrived at 1:30 PM and departed at 3:40 PM.

Violations:
Standard #: 8VAC20-770-60-B
Description: Based on review of staff records, the center failed to obtain that all staff had a completed sworn statement.

Evidence:

Staff # 5 had a hire date of 01/08/2024. A completed sworn statement was not in the
record. Staff # 7 did not provide documentation when requested.

Plan of Correction: Staff completed the Sworn Statement during the inspection and director will make sure that all required forms are completed.

Standard #: 8VAC20-780-60-A
Description: 2nd Repeat Violation

Based on review of eight children?s records, the center failed to ensure that all records were complete per the standard.

Evidence:

1.The record for Child # 4 had documentation of the first day of attendance of 09/02/2019 was missing the address of one to the two emergency contacts, allergies and intolerance to mediation or any other substances, and actions to take in an emergency situation.

Plan of Correction: The director will obtain the missing information and will monitor all records.

Standard #: 8VAC20-780-560-G
Description: Based on observation of the refrigerator in the infant room, the center failed to ensure that all food brought from home was not dated and labeled with the owner?s name.

Evidence:
There was an opened jar of baby food that was not dated.

Plan of Correction: The baby food jar was labeled with the date during the inspection.
