Technical Assistance:
Reviewed all paperwork and notices regarding the elevator.

Reviewed all notices regarding Fire Department and notice of next fire inspection.

Comments:
An unannounced monitoring inspection was conducted today. There were 93 children present with 16 staff members supervision. Ratio's and supervision were in compliance. The physical plant, medications, medication authorizations, emergency supplies and procedures, evacuation logs, attendance logs, accident logs, children's records, staff records, hand washing, diaper changing and menu's were reviewed. The children were observed during playground free play, eating lunch which consisted of chicken, noodles, salad and milk, reading books and preparing for nap. Standard #: 8VAC20-780-60-A
Description: Based on a review of children's records, the center failed to ensure that all required documents are included in each enrolled child's records.

Evidence:
Child # 3's record does not have documentation of addresses for emergency contacts.

Plan of Correction: I will contact the parents and request the information.

Standard #: 8VAC20-780-560-F
Description: Based on a review of the center menu, the center failed to ensure that when the center chooses to provide meals or snacks that the center shall follow the most recent, age-appropriate nutritional requirements of the Child and Adult Care Food Program of the United States Department of Agriculture.

Evidence:
Morning Snack (Breakfast) for this day was Honey Wheat Bread and Butter. Please see attached United States Department of Agriculture Meal Plan for each age group of children served

Plan of Correction: The center will follow the USDA approved meal plan
