Technical Assistance:
Technical assistance was provided regarding labeling and storing staff?s personal items in areas that are not used to store children's items.

Comments:
An unannounced monitoring inspection was conducted on May 6, 2024, from 2:00pm to 3:05pm. At the time of the inspection there were 62 children in care and 10 staff members. Children were observed participating in various activities including nap, handwashing, and learning center play activities. Records were reviewed for five children and three staff members. Evacuation drills, emergency supplies and other required records and postings were reviewed. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed with the director during the exit interview.

Description: Based on observation and interview, the center did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a safe and operable condition.
Evidence:
1. The wood chips on the outdoor playground were not redistributed evenly to create resilient surfacing and the dirt on the ground was exposed at the foot of the play structure slide.
2. Staff #1 confirmed that the wood chips were not covering the ground at the end of the slide.

Description: Based on observation and interview, the center did not ensure that hazardous substances are kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. An unlocked cabinet in the four-year-old classroom contained a bottle of hand sanitizer which was labeled ?keep out of reach of children.?
2. Staff #1 confirmed the cabinet in the four-year-old room did not use a safe locking method that prevented access by children.
