Comments:
An unannounced monitoring inspection was conducted from 10:00 a.m. - 12:15 p.m. There were six children in care with the provider and one assistant, ages 17 months, 2 years, 3 years, 4 years, 5 years, and 6 years for a total of 11 points. The children were observed participating in center play, music and movement activities, and eating a morning snack. The morning snack served today is cheddar crackers, string cheese, and water. Six children's records were reviewed. The provider, one assistant, and one household member records were reviewed and in compliance. There are reportedly no medications being administered to children in the family day home at this time. The most recent emergency escape drill was conducted 07/17/2019 and the most recent shelter in place drill was conducted 06/10/2019. Based on review of six children's records it was determined that not all sunscreen forms were updated annually. Evidence: The most recent sunscreen form for child B and Child C were dated 06/24/2018.

The provider will update the sunscreen forms for child B and child C.

Based on review of six children's records it was determined that not all records contained documentation of a physical within the first 30 days of enrollment. Evidence: Child A with a start date of 06/22/2019 did not have documentation of a physical on file.

Based on review of six children's records it was determined that not all records contained documentation of updated immunizations every six months. Evidence: The most recent immunization record on file for Child B (date of birth: 01/02/2016) was dated 10/14/2016.

The provider will request the most recent immunization record from the parents of Child B.
