An unannounced complaint investigation was initiated on 11/29/21 and concluded on 1/27/22 in response to a complaint received in the licensing office regarding an allegation of a forbidden action. Based on program observation, interviews conducted, and information received from a local agency, there was insufficient evidence to support the allegation and the complaint was determined to be invalid. An exit meeting was conducted virtually with the provider on 1/31/22.
